Individual Health Care Plan (IHCP)
At the time of the visit four children with emergency medications were reviewed and the following was found: one child did not have one of their prescribed medications onsite.
At the time of the visit four children with emergency medical conditions were reviewed and the following was found: one child was missing and IHCP form.
At the time of the visit three staff files were reviewed and the following was found: two files were missing staff information forms, one file was missing evidence of MMR, and one file was missing evidence of a current physical.
Injury log has not been accurately maintained. Since beginning parent communication via an app, injury reports have not been completed and logged in a timely manner.

A review of three children's records indicated the following: -Two files lacked a current Developmental History and Background Information Form. -Two files lacked a complete Individual Health Care Plan (IHCP). All IHCP's must include the name of the educator(s) that received training addressing the medical condition and the name of the individual that conducted the training.
