The following was observed to be accessible to children during the visit: in the Community Room used by the School Age program, the kids' couch had a long tear on the seat.
Two IHCPs for School Age children were reviewed and found neither form was complete. One IHCP was missing side effects of treatment, symptoms, and name of chronic condition and the second IHCP was missing side effects of treatment.
In School Age, one child's antihistamine was not present at the program.

Four staff records were reviewed (2 from the child care program and 2 from the out of school program) and found: One staff record did not have evidence of immunity to MMR and one staff record had an expired PQ registration (expired 1/4/2025) and was missing bi-monthly observations (none between 2/1/2024 and 10/31/2024).

Medication
The evacuation procedures were missing from the threshold that connects the Preschool and the PreK classrooms. This should be posted on both sides. The evacuation procedures were also missing from the door that leads from the Toddler room to the Infant room.
The evacuation drill log was reviewed and found to be complete. The second exit in the INFANT classroom was obstructed by a gate, a chair and a baby bouncer. All exits and evacuation routes must be kept clear of obstructions.

The black fencing installed around the outdoor playground was observed to be an entrapment hazard due to the space between the vertical rungs. An entrapment hazard is space measuring between 3.5 and 9 inches. The space between the fencing rungs measured at approximately 3.75 inches and approximately 4.5 inches in two spots where the fence is bent (near the entrance of the playground area).

Attendance in the INFANT/TODDLER room was not consistent with the number of children in the room during the visit. The attendance observed indicated six children present but nine children were present.
A review of 20 staff files revealed: -One did not include documentation of First Aid training. -One did not have a required resume or job application. - One did not have documentation of required reference checks. -One did not have evidence of a Staff Orientation.
A cleaning dust pan was on the floor in the Toddler classroom bathroom within the reach of children.

A review of four children's files indicated that one Individual Health Care Plan (IHCP) was incomplete. The plan did not include the person who trained the educator and which educator was trained to address the child's medical condition.
