Medication
Two children's antihistamines required to treat minor symptoms of allergies were not immediately available. One child's parents did not provide it to the program and one classroom did not take the medication outside along with the class.
On the morning of the visit, a child was bit on the finger by a mouse in the classroom. The interior of the child care program must be clean and maintained free from vermin.
The following was observed during the visit: In BUSY BEES (Infant/Toddler), the second teacher left the classroom to fill up the water pitcher, leaving one teacher with six children and in PRESCHOOL, the second teacher left the classroom to use the bathroom, leaving one teacher with 16 children. A music class was occurring so there was two additional adults in the PRESCHOOL classroom but they are not employed by the program and do not count in ratio.
The following was observed to be accessible to children: In PRESCHOOL, the blue wall under the window looking into the PreK classroom has peeling paint.
Four staff records were reviewed and found one was missing evidence of immunity to MMR and three were missing bi-monthly observations.

The Evacuation Drill Log was reviewed and found drills in the past twelve months did not have the number of children recorded.
The following was found to be accessible to children: -In the OPEN/CLOSE classroom, the vertical bookshelf was not secure and tippable. -In JR. K classroom, the play kitchen set and tall reddish brown shelf below the air conditioner was not secure and tippable.

A review of three staff records indicated the following: -Two did not have documentation of a current physical examination. -Two did not have evidence of (REDACTED) -One did not have documentation of an interview. -One did not register annually with the Department of Early Education and Care.
In the Pre-K Classroom, the cord to the CD Player, longer than six inches in length, was lying on the floor. Strings and cords longer than six inches that are not part of recreational or educational materials, including, but not limited to cords on window blinds, curtains or shades, must be kept out of children’s reach.
A review of three Individual Health Care Plans indicated the following: -One did not include documentation of the consequences of the medication not being administered

Three staff records were reviewed and found: -One staff record did not have a current physical and was missing bi-monthly observations since 12/13/2022. -One staff record did not have bi-monthly observations since 5/9/2023 and the Orientation Checklist was incomplete. -One staff record did not have a current physical and did not have any bi-monthly observations since being hired in 9/2023.
Four children's records were reviewed and found: -Three child records were missing medication consent forms.
The attendance in the Infant/Toddler Classroom was not accurate during the visit. Nine children were marked as present but two were in another classroom. The attendance was updated during this visit.
Unknown vegetation was observed to be growing through the fence in the Toddler Play Area.
Antihistamine for two children were not readily available for staff. One was located in the Director's office area and the other had expired and the Director is waiting on the parents to provide a new one.
