The pack n play for the infant has a hole in it and is not in good repair. The pack n play sheet for the infant is not tight fitting. The educator changed pack n play to a new one which was in good repair and placed a tight-fitting sheet on it during the visit.

The First Floor Kitchen cabinets, doorway and molding of the Dining Room, the top step of the basement stairway, the door leading to the Basement, the walls of the basement stairway, the columns by the front door of the house on the first floor, and the posts outside of the first floor first exit have chipping and peeling paint.
The post of the gate to the outdoor space is not secured and could be a tipping hazard to children. The outdoor stairway leading to the first level of the outdoor space from the driveway, is missing a handrail.

The licensor observed: The lattice on the railing is broken and insecure, posing a fall hazard. Garden hose across the area near stairs to the lower-level play area, posing a hazard. The gate to the lower-level play area is not secure causing the fence to sway posing a hazard.
The educator was missing the number for the Board of Health on the required posting.
