Playrooms: the toys shelves are not firmly secured to the wall. The educator is using a cabinet latch to secure them to the wall, and they are still tippable. Kitchen: the drawers are not child proofed and licensor observed medicines and sharp tools stored in them. Bathroom: licensor observed a bottle of Fabuloso Detergent stored in the open area within children's reach.

The back porch stabilizers, railing on the second floor back porch, the eves under the roof around the home, all of the window frames outside the home, the top of the sides of the front porch, the stairs leading to the front porch, the door frame at the back exit, the door frame at the front exit, the area to the right of the front door where the home meets the porch, and the molding in the Bedroom all have chipping and peeling paint.
There is a large wooden swing set with a tall slide and climbing wall in the outdoor space which does not have adequate fall zones or protective surfacing.
