The educator has two First Aid Kit, one stays in the program with the educator staying indoors with the children and one goes out with the educator taking the children outdoors. One of the First Aid kit is missing the CPR mouth guard, the thermometer, and the children's emergency cards.
Per refrigerator thermometer, the refrigerator temperature is 50 degrees Fahrenheit.
One child was left outside for a brief moment when the educator and the children returned from the library. The educator noticed when the child knocked on the door.

The Licensor observed plastic bags on a kids' picnic table that could be a suffocation hazard to children. The licensor observed metal chairs stacked against the wall outside that poses a tipping hazard for the children as well as a safety hazard, children could climb them and risk falling. TA trampoline was observed in the backyard. Landscape and construction materials (ladder, wheelbarrow, tiles) accessible to children posing a safety hazard. A rusty chair in the center of the yard presents a safety hazard to children. A firepit with a sharp tool placed on top presents a safety hazard to children. Tree branches scattered on the backyard floor create a tripping hazard. A loose piece of wood on top of the playhouse presents a sharp object risk.

There is an open container of disinfecting wipes, and a bottle of hand sanitizer on the bathroom counter, which are accessible to children and could be hazardous. At the time of the visit, the educator moved the wipes and the hand sanitizer to a high shelf, so it is no longer accessible to the children.

There is a pair of adult scissors on the dresser in Nap Room 2 which are accessible to children and could be hazardous. There are 3 knives, a lighter, 2 can openers, a pair of adult scissors, 6 steak knives, and a bottle opener in a drawer in the Kitchen, which are accessible to children and could be hazardous.
Balcony does not have a barrier of 7ft high around it.

ER exit blocked. Evacuation drill unsuccessful. The licensor noticed an exit sign on the door leading to the stairs to the second floor, but the sign does not clearly indicate the location of the exit. Additionally, the licensor was unable to confirm the exit to the outside, as the exit was inaccessible.
6 outlets uncovered and pose a hazard to the children. Water temperature is 128 degrees Fahrenheit. Licensor observed a gate at the door leading to the kitchen in the basement; one of the wooden slats is loose, creating a gap, which poses an entrapment hazard.
