The 2 certified assistants were observed leaving 2 children alone, unsupervised in the Kitchen, while they were in the Infant Room and Playroom and could not see the children. The door to the Infant Room was shut while the 3 Infants were sleeping, which was discussed at the previous visit.
Cots, portacribs, and pack n plays, are not labeled.
Assistant was observed leaving the door to the Napping Room shut where 3 children were napping.
Second exit route is obstructed by: a tall shelf at the bottom of the stairs, which blocks the gate door from opening all of the way, 2 folding chairs, and 3 bottles of laundry detergent, a small rug rolled up, a large bottle of bleach, 4 boxes and a stack of books were on the stairway leading to the first floor.
