All of the children's records were reviewed. Three children required an annual update form, one child required an updated physical and immunizations and two children required corrected emergency permission sections.
This investigation inspection recorded no violations or advisories.
The fence had a section of panels which were broken. The wooden picnic table was broken and had sharp wooden pieces.
One of the kitchen's in the drawer was unlocked allowing access to sharp kitchen utensils. The bathroom cabinet was also unlocked allowing access to hazards. Two electrical outlets were uncovered in licensed space.

There were three fence panels on the backyard that were not secured and post a hazard to children.
There was one drawer in the kitchen with batteries and seeds that were accessible to children. The seeds were large enough to place a choking hazard.
Two children were missing a progress report.

Records were reviewed for all children and four children had incomplete enrollment packets. Five children were missing proof of current medical documentation.
Indoor Hazards
The Licensor observed the cable cords accessible in the living room. The Licensor observed 2 small tables in the living room that were not bolted and could pose a tipping hazard. The changing table isn't bolted. Creams, lotions and plastic gloves were accessible in the bathroom. Shampoos were also accessible in the bathroom.

In reviewing the provider's evacuation logs, she is not alternating exits on a monthly basis while conducting the evacuation drills.
