The certified assistant was staffing the program, providing care in the educator's absence. This assistant has exceeded the maximum of twenty-five hours in a twelve-month period. This certified assistant license does not meet the same qualifications as the educator's.
An (REDACTED) infant was observed asleep behind a closed door in a separate room. When children are placed in a separate room for naps, the door must remain ajar.
The pack and play identified for napping was observed to have various play materials inside.
The child was observed to be sleeping in an inclined position. The child was immediately removed by the assistant upon licensor observing.

In the playroom, an extension cord with several electrical outlets were accessible to the children. Two topical ointments were on top of the diaper changing station accessible to the children. The educator has requested to license the kitchen in which the following hazards were observed in the kitchen drawers which were accessible, tile grout, plastic bags, sharp utensils, scissors, a screw driver, grater and a blender with a blade.

In the bathroom, the cabinet under the sink was unlocked allowing access to cleaning supplies. The base heater in the bathroom was rusted.

The first aid kit was missing the children’s emergency cards.
All of the children's records were reviewed, four children need updated medical physicals, one child needs page six and seven of the enrollment packet, one child needs an annual update form, and one child has several sections of the enrollment packet not completed.
Several toys were observed to have sitting water in them and an umbrella with several loose pieces was on the floor.
The assistant has not documented their in/out time for staffing the program since 04/01/2026.

Two children did not have a date of admission on their enrollment packet, one child did not have page six and seven completed, one child did not have page seven completed, three children did not have annual enrollment forms, three children did not have up to date physicals and one child did not have lead testing. The medication consent on file for one child was not the correct consent, did not include the parent's signature or date.
All of the progress reports were reviewed, one child did not have a progress report, one child's progress report was over due and one child's progress report was not dated.
The slide in the outdoor play space measured over 18 inches in height. The slide did not have an approved fall zone.

All of the children's records were reviewed. Two children require an annual update form, two children required proof of immunizations, one child had several sections of their enrollment packet incomplete and one child did not have proof of lead testing.
The educator wants to add the kitchen to their license. There were several drawers in the kitchen which had sharp knives, utensils and medication accessible. The oven knobs were not covered. In the bedroom, a wire which measured more than six inches was not secured.
The children's allergy posting did not include the name of the emergency medication, the location of the emergency medication and was not in a manner that protects the privacy of the child.
The back exit in the playroom was being obstructed by a large chair. The chair covered the entire exit, not allowing access.
