In the outdoor area several toys had sitting water/ice and a bags of screws was on the ground. The educator was observed to remove the bag during the visit. The water temperature was tested and measured over 120 degrees.
An (REDACTED) was observed to be in the rocker holding and feeding themselves a bottle.
An (REDACTED) was observed to be sleeping in a crib with a large blanket, a smaller blanket and two stuffed animals. The educator was asked to remove all of the items in the crib.

The educator has a fenced in area which measures 600 square feet. The outdoor area doesn't meet the square footage requirement of 750 square feet.
In the outdoor play area, the fence did not meet the four-foot height requirement, the fencing was risen allowing a seven-inch opening on the bottom posing an entrapment hazard, there were loose rocks, uneven grounding, loose wood planks, PVC trim, a rake, a shovel and a bag of screws accessible.
In the playroom, licensor observed accessible wipes, gloves, plastic trash bags, and two uncovered electrical outlets. IIn the living room, the licensor observed a wired vacuum being charged and accessible to the children.

The first aid kit was missing one of the children's emergency cards.
In the kitchen, one cabinet had a safety lock which was not functioning allowing access to cleaning supplies under the sink.
One of the diapering surfaces was of cloth materials. The material must be able to be disinfected or disposed of after each change.

There was sitting water in the play kitchen, an uncovered gas grill with gas tanks and a slide which was over the allowed 18 inches without an appropriate fall zone.

One of the children’s enrollment packet was not entirely completed. One of the children was missing a lead test and there were several children whose transportation and emergency permission section in the enrollment packet was not complete.
The water temperature in the bathroom measured at 130 degrees, over the allowed 120 degrees.
There was a phone charger wire in the licensed bedroom accessible to the children.
