This follow-up visit inspection recorded no violations or advisories.
The assistant was observed to leave three children inside of the home when transitioning to outdoor play and then leaving two of the children outdoors while returning inside to bring two of the remaining children outside. The assistant then left the infant inside while bringing the other two children outside. When the assistant returned inside to bring the baby outside they allowed the four other children to remain outdoor unsupervised.

The (REDACTED) was observed to be sleeping in another room separate from the assistant who was staffing the program not allowing the assistant to have direct supervision of the infant. The assistant had removed the child from the playpen was going to place the child in the car seat outside while napping. The assistant was informed they could not per safe sleep policy. Children younger than six months of age at the time of enrollment must be under direct visual supervision at all times, including while napping, during the first six weeks they are in care.

All of the children’s records were reviewed. One child needs an annual update form.
The assistant was not observed to use appropriate child guidance during on of their interactions with the children. The assistant responded "you shut up" to a child after a child was repeating "shut up."
The educator did not have a staff record for their assistant that included documentation of CPR/First Aid training, health records, or documentation of required training.
The regular assistant reported that they solely staff the program on Mondays and Wednesdays from 9:00 am to 12:30 pm. The assistant's attendance reflects that they worked on 09/10, 09/15, 09/17, 09/18, 09/22, 09/24, and were staffing the program today. The assistant has been alone on the premises with up to six children for more than eight hours in a seven day period based off their attendance and what they reported during the visit.
The dog's license with the town was not in the pet's folder. The vaccinations were up to date.
The borders of the inside of the exit door had exposed insulation.

Progress Reports
In the kitchen, there were several drawers with no safety lock allowing access to lighters, small coins and wires longer than six inches. In the bathroom, there were several drawers without locks allowing access to make up, personal hygiene items and razors. The heater in the bathroom had a section with rust. In the living room, a play kitchen and toy shelf were not secured posing a tipping hazard.

All of the children's records were reviewed, two children did not have the general permission section completed and one child did not have a date of admission documented.
The educator is not alternating exits on a monthly basis when conducting evacuation drills.
The educator has one dog whose vaccines are currently expired.
In the outdoor space, there were several toys with sitting water.
