A prescribed medication for a child had expired in June of 2024. Medications must be appropriately stored, labeled and not expired.
At the time of the visit, the program did not have a current Medication Consent Form for a child with a prescribed medication. Written parental consent shall be valid for one year, unless withdrawn sooner. Such consent and authorization must be renewed annually for administration of medication.
Plan for meeting potential emergencies
Observations from original visit on 2024-02-26 00:00:00: The program must update their policies to include potential emergencies including but not limited to missing children, the evacuation of children from the program in the event of a fire, natural disaster, loss of power, heat or hot water or other emergency situation. Corrective Plan received on 2024-03-22 00:00:00: The program will create a policy to align with the EEC regulations for each required area.
A review of three Staff Records during the visit indicated the following: -Three educators had evidence of some, but not all bi-monthly observations.

The following was observed to be accessible to children: -In the classroom, a metal bracket attached to the left end of the long red bookshelf was sticking out and had a sharp edge. -All furniture in the classroom not on wheels was not secure and tippable. -In the "library", a screwdriver was observed to be sitting on top of the bookshelf to the left inside the room; a container of other tools including a hammer, paint scraper, and metal tape measure sat on a pile next to the bookshelf with the screwdriver; and all of the furniture against the walls was not secure and tippable. -In the back office, two staff pocketbooks were observed to be sitting on a chair next to the desk. One appeared to be unzipped. -The wooden bookshelf in the office at the back near the exit door was not secure and tippable. -In the staff bathroom, two plungers and a toilet brush sat on the floor next to the toilet and an aerosol air fresher sat on the windowsill.

Toxic Substances
The Staff Schedule was posted but under multiple papers and was not visible to visitors and parents as required.
The evacuation procedure was not posted at the back door exit.
The evacuation drill log was provided but upon review it was found drills conducted in January and February 2023 were not included.
The closet off of the hallway was open and observed to contain the following accessible to children: - A bottle of bleach, two spray cleaning products, a can of Ajax, a bottle of Lysol and a bottle of dish soap all with the label of "Keep out of reach of children".
It was identified through the review of the Allergy List that a child in the room has an allergy that required an (REDACTED). Further review of the child's record confirmed the child is prescribed an EpiPen. The Allergy List was posted on the wall but the medication was not on the property. Emergency medications such as epinephrine auto-injectors must be immediately available for use as needed.
