A prescribed medication for a child was not in the containers in which they were originally dispensed and with their original labels affixed.
The educator had not documented the time in/time out for the assistant staffing the program. The educator had not documented the time out of the assistant who had left the program for a period of time during the day.

Five of the ten children's records were reviewed. One child did not have an annual update form, One child did not have their admission date on their enrollment form and their permission for travel was signed authorizing to leave the premises and not to leave the premises.
The water temperature was tested and measured at 135 degrees, over the allowed maximum 120 degrees.
There are two children enrolled with a medication allergy. The provider did not have the child specific required posting.
One child who was present for his first day did not have a completed enrollment form, two children were in need of an annual review form and one children required CPR and general permission authorization completed.

The Licensor observed a small trampoline in the outdoor play area. Trampolines are not currently allowed in family child care programs.
The Educator is using one diapering pad during diapering when two items are currently required.

Provider has a slide over 18 inches which did not have an appropriate fall zone.
In the outdoor space, the deck had a broken piece of wood posing a safety hazard to the children. The water hose was unraveled. The surrounding fence of the play area had broken pieces.
In the kitchen there was a drawer with phone charging wires and batteries accessible to the children.
