A box of screws were accessible in the kitchen and a tube of baby ointment was accessible under the diaper changing station.

Provider did not have updated progress reports for two children.
In the play space in the basement, there are two shelves which are not secured and are a tipping hazard. In the outdoor space, there were some toys with water sitting in them and a slide over the allowed 18 inches.
Provider was observed to be changing a child’s diaper without a second surface which is able to be disposed of or sanitized.
There was a slide set higher that 18 inches that did not have the required ground surface and the 6 feet in all directions from the perimeter of the equipment.
