Children’s records had not been updated annually. There was no documentation of current physicals and immunizations in three of the four records reviewed.
One child’s allergy was not posted. There was no Health Care Plan completed and on file for the child with the allergy.
Provider’s FA and CPR had expired 8/28/23.
Upon arrival at the program there were children resting in front of the exit. Exits must remain clear at all times.
There was a medicine chest near the back entrance that was not locked and was accessible.
