Water Temperature
All of the children's progress reports were reviewed. There are two children who need an updated progress report and two children who did not have one on file.
All of the children's records were reviewed. Five of the children needed an updated physical and one child needed proof of lead tested and updated vaccinations.
The water temperature measured over 120 degrees.

First Aid Kit
All of the children's records were reviewed, two children's enrollement packets were not fully completed, four children did not have an annual update form, one child did not have their vaccination records, two children did not have their lead testing and one child had an overdue physical.
Several of the children's emergency cards in the first aid kit were expired.
In the outdoor space, the outdoor play kitchen had sitting water and the play house had chipping and peeling paint.
Indoor Hazards
The educator was observed to use a microwave to warm a bottle. The use of a microwave for bottle warming is prohibited.
Injury Prevention
In the playroom, the gloves on the diapering table were accessible. The window blind was observed to be broken. In the kitchen, trash bags were accessible under the sink and the oven knobs were accessible.

In the dining room area, a section of the floor near the exit is lifting. A cabinet in the kitchen was not locked allowing access to dishwasher pods and cleaning supplies. In the bathroom, a section of the bathroom is being remodeled and allowing access to a large hole in the floor.

Licensor reviewed the evacuation log, provider is not documenting the length of time of the drill or the accurate meeting place.
This follow-up visit inspection recorded no violations or advisories.
One toy shelf in the playroom was not secured to the wall posing a tipping hazard. A cabinet in the kitchen had a blender with a sharp blade accessible to the children.

The provider did not have progress reports for any of the children enrolled.
All five of the enrolled children's record's were reviewed. Three of the children's general permission sections were not fully complete, one child did not have their lead testing and one child's entire enrollment packet was not complete.
The first aid kit was missing the children's emergency medical cards.

One (REDACTED) was observed to be sleeping in a bedroom which is not licensed space.
The second exit had various hazards obstructing the pathway including a water hose, wood and building materials.
The provider does not have a second diapering surface able to be disinfected or disposed of.

The infant was observed to have a bottle in his pack and play. Provider reported he will not drink his bottle while being held.
