Three of the children's progress reports were reviewed. One child needs an updated progress report.
Three of the children's records were reviewed, one child needs an annual update form and another child did not have the general permission section completed on the enrollment form.

The educator did not document time out for two children yesterday 10/01 and there was no attendance documented for today. The educator is not documenting the time in/time out of one of the children and only documenting if the child was present or absent.

In the nap room, there are several furniture pieces and napping materials which do not allow for open activity space for the children. This activity space is needed to be able to upgrade the licensed space to ten children. Items will need to be removed to allow for activity space.
This investigation inspection recorded no violations or advisories.
In the bathroom, the phone charger was plugged in loose and accessible and the cabinet under the sink had a safety lock which was unlocked allowing access to the hazards.

The Licensor observed the refrigerator cord and microwave cord accessible in the kitchen area.
The Educator installed a new swing set. One side doesn't have the six foot clearance, as required. Additionally, there are small rocks accessible in the outdoor play area. The rocks are chokable.

The educator had not documented the children's attendance on this day of the visit.
The educator identified an individual who was present during the visit as regularly on the premises. This individual did not have a background record check complete.
The educator did not document a fire drill on the fire drill log for the month of April and the educator is not alternating exits on a monthly basis.
The first aid kit was missing two of the children's medical emergency cards and all of the cards in the kit were expired.
There were several electrical outlets in the play space were uncovered, the educator covered them during the visit. In the outdoor space, there was no gate on the stairway allowing access to the deck, several wood panels in which the children were able to climb on and a rusted rake leaning against the fence.

The children's records were observed. In observing the children's records, two children had an overdue physical, one child was missing lead testing and two children had missing information on the transportation permission.
Provider is not alternating exits every month and not documenting a meeting place.
Provider's handbook did not have all of the required information that is included in the EEC handbook.
Provider did not have a carbon monoxide detector on the first floor.
Provider did not have her smoke detector connected on the first floor of her home. Provider did connect it during the visit.
Provider had shelves in the nap room which were not secured causing a tipping hazard. The closet door was not locked allowing access to laundry detergent, an air fresher was plugged into an electrical outlet accessible to the children. Provider removed the air freshener during the time of the visit and covered the outlet with an outlet cover.
One child has an (REDACTED)allergy. Provider does not have the child specific posting of the allergy.
