Records were reviewed for the five enrolled children. Two children were missing Immunization updates. One child was missing annual update. Two children's enrollment forms had incomplete information under "Permissions" and "Emergency Card".
The following safety hazards were observed and accessible to the children: BATHROOM: - Listerine and body massage lotion on top of the sink - Air freshener - Wipes - Unsecured bathroom cabinets containing hazardous items, - including but not limited to deodorant, perfume, body lotion, oral rinse, and hair products LIVING ROOM: Two uncovered outlets Several hazardous items inside the console table drawers, including but not limited to an open VapoRub container, fish food, fish tank water solution, computer chargers, hand lotions, and a large tool KITCHEN: Ice maker machine cord connected with the wire hanging UNDER THE CHANGING TABLE: Accessible wipes and gloves
Under the diapering station there was a small bottle of nose saline spray accessible.

All of the children's progress reports were reviewed. Three of the children's progress reports need be updated.

The children's records for the two new enrolled children were reviewed, one child did not have a completed enrollment packet.
This monitoring outside differential licensing cycle visit inspection recorded no violations or advisories.
Safe Sleep - no pillows, comforters, stuffed animals
The infant was observed to be asleep with a Phillips AVENT Ultra Soft Snuggle Pacifier Holder in the playpen next to them.
Equipment, materials and furnishings - Play Materials
A (REDACTED) was observed to be napping in the playpen with a pillow, play mat, toys and a pacifier cord with a clip loose.
In the kitchen, several drawers and cabinets were not locked allowing access to medication, batteries, a food processor and loose cords longer than six inches. In the living room, there was an air freshener, several loose electrical cords, fish food and cleaner all accessible to the children.

The outdoor space has several wood palates and several long wood pieces not secured on a wood table accessible to the children.
