In the outdoor space, several toys had sitting water, the water hose was unraveled, a section of the fencing was broken and the gate at the bottom of the stairway was broken.
In the bathroom an air freshener was accessible, the air freshener read "keep out of reach of children." The book shelf was not secured posing a tipping hazard. A section of the floor in the second living room was broken. There were several pieces of broken plaster and peeling and chipping paint on the walls in the licensed space.

All of the progress reports were reviewed, one child did not have a completed progress report and the four others were completed but no documented date of completion.

The first aid kit did not include two of the children's emergency medical cards.
