The assistant did not wash and disinfect the changing surface after diapering a child. A protective covering was not used on the changing pad for each diapering.

There was rust on the edges of the bulkhead exit that was accessible. The corner of the granite counter top in the child care space was protruding and was not covered. The posts in the basement child care space were not covered with a protective material.
