During the visit, the Licensor observed an (REDACTED) child seated in a highchair that was missing a safety strap. The child was unstrapped and attempted to climb out of the chair.

Provider’s CPR/First Aid expired on 02/21/2024. The provider's assistant has current CPR/First Aid which expires on 2/27/2025.

In the infant/toddler area, the grey foaming on the wall was observed to be ripped posing a chocking hazard.
This follow-up visit inspection recorded no violations or advisories.

A (REDACTED) infant was observed to be asleep in the bouncer with a bib and a pacifier with a string attached to his bib.
In reviewing a select number of children's records, one child is missing a physical, immunizations and lead testing and another child is in need of an updated physical. Provider reported both of these children have their physicals scheduled.

Blind strings in the playroom were hanging within reach of children. A liquid air freshener jar was within reach of children near the bathroom sink.

The Licensor observed loose camera cords and television cords in the living room. The cords could pose a strangulation hazard. Two desks located in the hallway contained hazards in the draws that were accessible. Diapering creams and plastic gloves were observed accessible in the diapering table. Two slides were observed on grass with no ground surfacing, as required. Small rocks were observed accessible in the play area. Trampoline poles and broken fence were observed outside the fenced area.
This investigation inspection recorded no violations or advisories.
A (REDACTED) was given a bottle to feed herself during nap time. Educator’s must hold infants while feeding a bottle.
