While the program had a completed Staff Record checklist, after further licensing review, it was identified that the information contained on the checklist for the level of staff certification and/or qualification at the program level was not accurate.
A review of Four Staff Records at the time of the visit indicated the following: -Three records did not have evidence of a physical exam and immunity to MMR as is required by GSA (Group and School Age) programs. -Four records did not have evidence of having an interview and reference checks.
-The Staff Schedule was posted in the program and was reflective of the staff that were onsite at the time of the visit. However, a licensing review following the visit identified that the certification and/or qualification level of 8 out of 9 staff as indicated on the schedule were inaccurate. Technical Assistance was provided to the Director during the visit regarding Teacher Qualifications and Minimum Hiring Requirements. A follow-up meeting will be held with administrative staff, the Licensor and the Licensing Supervisor on 2/5/26 to address this.
The preschool classroom had an aquarium located on a low shelf accessible to children. The aquarium must not be tippable and must have a border or other mechanism around it to prevent children from being able to possibly tip it over. The Director did move the aquarium to the office at the time of the visit.
The program had a posted medication order for a child in the preschool class. This was posted in a manner that did not protect the privacy of the child. The program did not have a posted Allergy list indicating allergies, medical information, special diets and/or emergency medications as is required.
Staff Records
A walk through of the facilities at the time of the visit indicated the following: -In the Infant/Toddler class, the diaper changing table had been moved since the time of the last licensing visit on 9/2/25. The changing table is now positioned that when an educator is changing a child, their back is to the rest of the class and they are therefore unable to appropriately supervise all children. The licensee must ensure that space is arranged to provide educators to properly supervise the children in their care. -In both the Infant/Toddler class and the Preschool class, locks/bolts were placed on the doors since the time of the last licensing visit. This means that in the event of a potential emergency, the children and staff could not easily access the exit which is a safety concern. The Licensor asked the Licensee during the visit if they consulted with the town Fire Inspector about these changes and they stated they had not. The Licensee stated they put the locks on the doors because they were afraid the children could easily open the door and go outside. The program is located in a high traffic area and they were concerned about this.
This follow-up visit inspection recorded no violations or advisories.
The Licensee provided a Dual License Administration plan on 8/28/25 for FCC and GSA Licensing Supervisors to review. On 8/29/25, the Licensee was informed that the plan needed to be revised to reflect sufficient staffing in each program and resubmitted for Licensing Supervisors review and approval.
The program did have a posted Staff Schedule for parents and visitors to see in the main lobby entrance. However, the Staff Schedule listed Assistant Teachers being left alone all day on Fridays in both the Infant/Toddler mixed class and the Preschool class. Assistant Teachers must not be left alone unsupervised with children at any time. In addition, four educators that were listed on the Staff Schedule previously for the mixed Toddler/Preschool class are no longer working at the program. During the visit, the Director stated that this room is not currently in operation.

Three Staff Records were reviewed at the time of the visit. One record did not include evidence of a current physical exam. The last exam for the staff member was dated 3/19/23.
The mixed Infant/Toddler class did not have a working sink adjacent to the diaper changing area. The program had a portable sink next to the diaper changing table but it was not set up and in working order.
