All of the children's records were reviewed. Two children need an updated physical and one child needed proof of lead testing.
The first aid kit was missing three of the children's emergency medical cards.
The attendance reflected no sign-out time for 12/30/2025, and no attendance was completed for the following dates: 12/31/2025, 01/02/2026, 01/05/202,6 and 01/06/2026.
The assistant's CPR/First Aid certification expired on 06/13/2025. The educator has been operating the program with an expired CPR/First Aid certificate since October 09, 2025.
The educator did not have the posting for a child with a medication allergy.
The bathroom trash can was not covered allowing access to the trash.
On the changing table, several lotions, diaper creams and hand sanitizer were in reach of the children. Several trash bags were accessible in one of the drawers in the kitchenette area.

Progress reports were reviewed for the six children present in the program during this visit. Four children need progress reports update.

Records were reviewed for the children present in the program. Three children's enrollment packets were missing information in the "General Permissions" section and on the "Emergency Card." Three children did not have updated medical information on file, and one child required an annual update.
The refrigerator was 26 degrees under the minimum 32 degrees.

The records of the children present were reviewed. One child's enrollment packet had several pages incomplete, one child does not have a medical physical on file and one child does not have their permission section complete.
All of the children's progress reports were reviewed, two children required an updated progress report.
The attendance log did not have the children's arrival time for today or departure time for yesterday 12/13/23. One child was not documented on the attendance for this entire week.
The (REDACTED) was observed to be contained in the walker for over thirty minutes. Licensor had to inform provider and assistant to allow the infant to move freely.
Provider was using a cloth covering as one of her diapering surfaces.

Provider is required to have thirty hours of ongoing professional development. Provider only has 25 hours.
Provider has not completed any evacuation drills since February 2022. Evacuation drills are required to be practiced monthly.

There were disinfectant wipes on the floor in the playroom as well as in the bathroom accessible to the children. In the bathroom there was also a plugged in air freshener and bags accessible. The carbon monoxide alarm was tested and not functioning. In the playroom there were loose wires accessible in the desk drawer. There were some cabinets and drawers under the sink in the playroom that were not locked and had cleaning supplies and a lighter accessible.
Provider had a wooden slat at the bottom of each pack and play under the mat where the infants sleep. The wooden slat is not part of the pack and pack.
All progress reports are overdue for the children who have been enrolled for more than 6 months.
