Upon a review of three Children's Records, the following was indicated: -One child's record was missing two questions on the Individual Health Care Plan (IHCP) of which educators in the program were trained in the child's medical condition and who trained those educators. -One child's record did not have evidence of written IHCPs for two separate medical diagnosis of an (REDACTED) for the child. -One child's record did not have written evidence of an IHCP for a diagnosis of (REDACTED).
