The records were reviewed for the four enrolled children. Two of the children needed proof of current medical documentation.
The exit to the backyard was not labeled and was not clear of obstruction. The snow had not been cleared and there was a shovel and a trash bin on the top step.

This investigation inspection recorded no violations or advisories.
There was a plastic slide structure on grass in the back yard. There was not adequate fall zone or protective ground cover under and around the equipment as required.

The licensor observed the heating vent in the playroom was missing, there was a mirror in the bathroom that was not secure and a shelf needs to be secured to the wall

There were plastic bags and wraps with sharp edges accessible in one of the kitchen drawers. There were leaf piles accessible around the outdoor space. There was one section of the fence near the driveway that was open and there was another section broken with splintery edges on the side. There was a wheel barrel full of water and debris accessible. There was a trampoline accessible in the outdoor space. There was chipping paint on the shed and on the back entry of the house accessible. The rope swing was accessible. There were hanging lights accessible by the house.

The trampoline entrance was open in the yard and accessible. The basketball hoop was ripped and within reach of children. The propane heater in the outdoor area was not secured out of reach.

Persons on Premises
Phyllis Huffam HHM was alone with children upon arrival. HHM did not have a current assistant certificate to work with the children.
The chairs used by the children at the children's table were too small and table was too high.
The first aid kit did not include current emergency forms for the children enrolled.
A relative of the provider was on the premises during the visit and regularly. However, the provider had not completed the BRC process for the relative.
Provider left premises and three children alone with an unapproved assistant.
