All of the children's records were reviewed, two children need proof of lead testing, one child needs proof of immunizations and two children need an annual update form.
In the bathroom, the cabinet under the sink had a safety lock but was not working. The following items were accessible, cleaning supplies, laundry supplies and person hygiene items. In the kitchen, a drawer was unlocked allowing access to two lighters. The lighters were removed from the drawer during the time of the visit. In the living room, two toy shelves were observed to have chipping and peeling paint.

All of the progress reports were reviewed. One child did not have an up to date progress report.
The outside of the home had construction occurring during daycare hours. In the front of the home, there were building materials and tools. In the outdoor space, there were no hand railings on the stairway, there was loose wiring on the side of the home exposed, ladders, a rake, gutters, building materials and pieces of insulation.

The water temperature exceed 120 degrees. It was 140 degrees.

12 children’s files were reviewed. Multiple files were missing updated physicals. Multiple enrolled packets were missing signatures and dates and had incomplete pages.

The Licensor observed two loose cords in the kitchen.
The Licensor observed plastic wrap and plastic bags in two kitchen draws accessible. Additionally, the Educator uses a small room for storage. The Licensor observed shelves that were not bolted and bleach bottles on the floor. The room is not locked. The Licensor explained that a lock should be placed so that the children don't have access to this room.

The first aid kit is missing a thermometer.
The first aid kit did not include a pair of scissors.
Two of the three exits were not accessible and or safe due to the construction occurring on the outside of the home.
The baseboard heater in the bathroom was rusted.
Two play kitchens were not secured causing a tipping hazard.
A vacuum cleaner and a broom were obstructing a clear path to the back exit.
Educator is documenting daily attendance for children. However, daily attendance for assistant doesn’t reflect actual time in and time out.
Five children’s emergency cards were missing from the First Aid Kit. Disposable non-latex gloves were missing from the First Aid Kit. The educator added the gloves to the kit during the visit.
