In the kitchen, two cabinets had safety locks but were not fully functioning allowing access to sharp utensils and a blender with a blade. A cleaning spray was on the kitchen floor accessible and a disinfectant spray was accessible in the bathroom.
The records for the five children present were reviewed. Three of the children's general permissions section were not fully completed, one child was missing an admission date in his enrollment packet, one child's physical did not have his name or date of physical on it and one child with a medical condition did not have an individualized health care plan completed.
