All of the children's records were reviewed, three children need an annual update form and two children need an updated physical.
In the bathroom, there was a bucket of used water with two towels. The educator removed this bucket into unlicensed space during the visit.
In the licensed space portions of the activity space are being occupied by furniture and equipment such as a refrigerator and a bed and small sofa which is only for adult use. These items are occupying the children's useable floor space.

The progress reports for the five children present were reviewed. Two of the children had progress reports completed but no date of completion, one child had a progress report completed but was not up to date and two children did not have completed progress reports.
The certified assistant was not able to provide the pets vaccination and license documentation.

Although the provider started a change in household transaction, she never submitted it for the household member who moved into the home in February 2022. Therefore this house hold member has not completed the background record check process.

Safe Sleep
One child who was not present was signed in on the attendance for the day. The assistant attendance did not have assistants time in and time out.
The provider and assistant did not hold the (REDACTED) when giving a bottle. The child was placed in a bouncy seat with the bottle.
The (REDACTED) was resting in port a crib appropriately at time of visit and woke up after the licensor's arrival. However, there was a thin mattress pad between the sheet and the port a crib mattress that had not been removed while the child was sleeping. One other port a crib had a thicker pad between the sheet and mattress.
An infant was observed sleeping in a playpen where the mattress was removed and a soft foam mattress was being used. Also, the infant had on a bib. Both the soft mattress and the bib could cause suffocation and is a danger to the infant while sleeping.

One smoke detector was removed from the dining room. There was medicine on the dining room table. There was a cord accessible in the dining room. There is a filtered water bowl for the cat that is accessible to the children. This water bowl is larger than usual and has a motor that filters the water.
The front exit was blocked by a playpen and the second exit is blocked by toys.

All of the children's records were reviewed. Two children did not have an annual update form, two children did not have up to date medical physicals and one child did not have proof of lead testing.
The educator reported having three household members over the age of fifteen years old residing in the home without a completed background record check.
The attendance log did not reflect the children present today. The educator is not accurately documenting the time in/time out of the school aged child.
The educator is not alternating exits on a monthly basis when conducting the evacuation drills.
The educator did not provide a written notification to the parents of the change in household composition.
The educator did not have their emergency preparedness plan in the parent handbook available for licensor to review.
In the licensed space, there were a pair of gloves and two bottles of lotion in reach of the children. The educator removed them during the time of the visit.
A container of sanitizer wipes were observed in the bathroom accessible to the children.
The provider does not have smoke or carbon monoxide detectors installed. The first aid kit did not have the children’s emergency medical cards, scissors or tweezers.
