The educator is not documenting the length of duration of the evacuation drills and is not alternating exits on a monthly basis. The educator did not have the exit sign posted on the exit in the kitchen.
A (REDACTED) was observed to be strapped into a rocking seat in the living room. The educator reported that they had placed the child there for a couple of minutes because the child was (REDACTED) child present. The licensor informed the educator that this was inappropriate behavior management and guidance, and was asked to remove the child from the seat, which they did. The educator did not guide the child positively and consistently, based on an understanding of the child's individual needs and development.

Three of the children’s records were reviewed, one child did not have a date of admission completed, general permission, emergency card and page three of the enrollment packet. In the living room, a toy shelf was not secured posing a tipping hazard, lotions were accessible on the shelf under the diaper changing table, the outdoor front door had a piece of broken panel and a container of laundry detergent and softener were accessible in the hallway.
The following hazards was observed; the carbon monoxide detector was not installed and the baseboard heater had two pieces broken on the end. The outdoor area was not assessed due to the recent snow fall.
