Five of the children's progress reports were reviewed, all of the children needed an update progress report.
The five children's records were reviewed, one child needs an annual update form and one child needs an updated physical.
In the playroom, two screws were protruding outside of the wall and a piece of the mirror on the wall was broken.
The flooring leading to the front door exit is unfinished posing a tripping hazard to the children.

The educator has active constructive occurring in the licensed space to include the living room/kitchen. The following hazards were observed, unfinished flooring, construction materials, tools, section of open ceiling exposing insulation, open access to unlicensed space which includes open walls and ceiling with insulation exposed and hanging wires. In the bathroom, a mop with sitting water was accessible and the trash can was rusted. The educator is licensed for ten children, even though the educator only has five children enrolled the current playroom space being solely utilized for the children does not measure enough for ten children in activity space. The educator would need to utilize the licensed living room for additional space which there is current construction occurring.
This re-activate provider visit inspection recorded no violations or advisories.

The educator proposed to license an additional room to the license. The enclosed deck is an addition to the home. The space is unfinished with exposed plywood and not furnished for child care.
The stairway in the front of the home and stairway on the proposed additional space did not have a handrail installed on the stairway.
There are sections of the home undergoing construction. The walls of the second floor were removed exposing the inside of the second floor home, there were building materials, insulation, woods, loose cords, ladders and tools accessible on the outside of the home.
Progress reports were reviewed the nine enrolled children and were not completed or up to date.

Records were reviewed for the nine enrolled children. All the nine children did not have their medical information (immunization and physical) up to date. Also, records were incomplete or missing Annual Updates, Permission (General Permission- (Basic Transport) and Permission - (Transport to Medical Facility and Receive Emergency Medical Treatment). One child was missing an enrollment packet. *Educator's yard is not approved for outdoor play. Educator has an Alternative Playspace Agreement currently in place. Permissions to take the children out of the premises must be complete and signed by the parents.

Educator did not notify the EEC about ongoing construction in the program. Upon the licensor's arrival, it was observed that construction was taking place both indoors and outdoors. During the visit, licensor was informed by the assistants that the construction had been ongoing for at least two months. According to EEC regulation 7.04(15)(f) - Notification of Change in Program Space, the licensee is required to notify the Department, when possible, at least 30 days prior to any changes in the space used by the program. If the licensee is unable to provide 30 days' notice, they must notify the Department by phone immediately upon learning of the impending change.

One kitchen draw latch wasn’t working exposing cords, tools and other hazards. Loose cords in the living room and in the basement level were observed. Loose cords can cause strangulation. Diapering creams and lotions were accessible in the basement level in bins. The diapering table wasn’t bolted and could cause a tipping hazard. The shelf where nap items are stored in the basement level wasn’t bolted and could cause a tipping hazard.

No individual or disposable towels were available in the bathroom during the visit. According to EEC regulation7.11(10)(c), Hands must be dried with individual or disposable towels or automatic hand blow-dryers. The use of common towels is prohibited.
The following hazards were observed and accessible to the children, including but not limited to: LIVING ROOM: - Empty and packed boxes were on the floor, posing a tipping hazard. - Packed boxes were placed on top of the shoe rack, creating a risk of falling onto children. - Dirty dog pads on the floor. - A security camera wire was plugged in, with the cord lying on the floor in the entrance pathway, posing a tripping hazard. - A large construction tool, which appeared to be a professional drill machine with a sharp and pointed end, was on the floor, posing a significant safety risk. - A vacuum cleaner accessible to the children. - A plastic bag containing trash was on the floor. BATHROOM: - A bucket containing a dirty mop was accessible. - A large, opened bag of ice melt salt was accessible.
Upon the Licensor's arrival, it was observed that construction was taking place in the outdoor space. Several hazards were observed in the front yard, which it's the pathway of both entrances to the program (the living room and playroom entrances). These hazards included, but were not limited: - Construction debris scattered throughout the yard. - The fence surrounding the outdoor play area was down and laying on top of the outdoor equipment obstructing the area. - A large hole was observed in the ground, where an addition to the house appears to be under construction. The hole leads to what seems to be a basement area.

The attendance log for today did not have the date and two children present were not documented on the attendance log. The attendance for the two assistants staffing the program was not documented for today.

The Licensor observed potty chairs in the basement level. There is no bathroom in the basement level. The Educator explained that after each use she would run upstairs to empty the potty chairs in the upstairs bathroom leaving the child care children alone in the basement level.
The immunization records for the Educator's dog are not up to date, as required.
The Licensor observed potty chairs in the basement level near the microwave. Toileting can’t be done near food preparation. Also, there is no toilet in the basement level to empty the potty chairs, once used. There is no privacy for potty training in the basement level.
One mat was observed ripped. The Educator will need 10 napping items to increase her licensing capacity.
Loose fencing with sharp edges was observed on the deck gate entrance.
The Licensor observed loose cords in the living room accessible to the children. The cords could pose a strangulation hazard.
