The educator did not have the required ten hours of ongoing professional development hours. The educator has four completed hours for the year.
The educator does not have a parental medication consent for a child with a life saving medication prescribed to them.
The backdoor screen which is being used as an exit is ripped and broken. In the bathroom, several of the child protection locks were not functioning allowing access to items in the drawers including personal hygiene items, lotions, and beauty products.
In the fenced in outdoor area, there were several toys with sitting water and leaves, a broken table and a soiled diaper. Outside of the fenced in area there was an uncovered grill and a glass beer bottle. These items are accessible in exiting the space on route to the outdoor fenced in area. The fenced in area does not have the height requirement of four feet which allows access to these hazards.
The exit route through the backdoor had pillows on the ground obstructing the route and plastic outdoor deflated toy.
The outdoor fencing does not meet the four foot requirement all around to obstruct hazards. Outside of the side that does not meet the square footage requirement there were hazards which included building materials, environmental hazards and sharp stones.

The bathroom has several drawers which were unlocked allowing access to hair, beauty and person hygiene products. The gate at the bottom of the stairs in the basement space is broken. The padding on the beams in the basement space were broken and ripping off the beams allowing the children to put them in their mouth.
The attendance was not documented for yesterday or today. There were several days in reviewing past attendance in which the sign out time was not documented.
In the stairways leading to the upstairs exit, two piles of clothing were on the stairway obstructing the exit route.
The records of the three children enrolled were reviewed. Two of the children did not have an admission date, one child did not have their lead testing and one child did not have their general permissions or emergency card completed.
This follow-up visit inspection recorded no violations or advisories.
The pack and plays were observed to have loose sheets, an infant lounger and a pacifier with a small plush toys hanging from the infant's pacifier. The provider reported the infants had slept prior to the visit with the pack and play as observed.

The emergency preparedness plan, on the parent handbook, was missing the plan for what to do if a child goes missing, and an alternative location, if the program needs be evacuated.
First aid kit was missing thermometer, CPR mouth guard, and children’s emergency cards. Educator added children’s emergency cards to the kit during the visit.

All of the children's progress reports were reviewed. One child requires an updated progress report.
The smoke detector in the basement did not have a battery installed.
The carbon monoxide detector in the basement did not have a battery installed.
The shed in the outdoor space has several broken pieces of wood, painting and chipping paint and a screw accessible to the children.
The provider has three children under the age of two years old enrolled. The three children are of the following ages 6 months, 4 months and 14 months. None of the three children are fifteen months and walking independently.
The provider is utilizing the same facilities used for hand washing after diapering or toileting as they use for food preparation and food service
The provider did not conduct and document and evacuation drill for the month of October.
One child enrolled did not have an enrollment packet with all required authorizations and information.
The emergency preparedness plan did not have an alternate location in case of an emergency. Provider did not have an exit sign on the backdoor exit.

The first floor did not have a fire alarm or carbon monoxide detector installed. The cabinet under the bathroom sink was not locked allowing access to cleaning supplies. The baseboard heater in the bathroom was rusted and broken.
