1. Documentation of a current physical examination was not available for review for a staff member hired on 11/18/2024. 2. Documentation of an additional 17.5 hours of annual professional development activities were not available for review for a staff member working 20 or more hours per week.
Documentation of an October 2024 staff meeting agenda was not available for review.
A background record check review reflected a staff member with an expired fingerprint status as of 11/22/2024.
Individual health care plans for three children diagnosed with a chronic medical condition did not reflect written parental consent.
Personal items intended for individual use (two children's water bottles) were not labeled with the name of the child for which they were intended to.
Diaper rash ointments and trash bags were stored inside an unlocked drawer in a restroom used by children.

Documentation of an updated physical examination and immunization records were not available for review in a child's record file.
1. Staff’s personal belongings (purses/bags) were stored inside an unlocked cabinet, accessible to children in a {REDACTED} classroom. 2. Adult size scissors were stored inside an unlocked countertop drawer, accessible to children in a preschool classroom. 3. Plastic bags were stored inside an unlocked diaper changing table, accessible to children in the {REDACTED} classroom.

Food packages were stored in a closet shelf next to cleaning products. Opened food packages were not stored in a covered container.
A child's {REDACTED} medication did not contain the original box and or an affixed label. Additional technical assistance is being provided regarding over counter medications noted on IHCP's.
A review of FY 2022 Emergency Evacuation observations reflected the following. 1. Documentation of an April and July 2022 practice drill were not available for review. 2. A practice drill conducted in March of 2022 did not include documentation of the number of children evacuated and the date.

Emergency evacuation observations were missing documentation for January through August of 2019. Completed monthly practiced fire drills for September through November 2019 were missing documentation for effectiveness.The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.

(2) staff files contained an expired CPR training certificate. CPR training must be renewed annually.
(1 ) personnel record had an expired physical dated: 12/1/2016. (1) staff record did not contain evidence of {REDACTED} and physical health records. (1) personnel record did not contain a date for one {REDACTED} immunization.
(1) child’s record did not contain evidence of {REDACTED} Screening. Copies of the childs immunization, physical examination and {REDACTED} records must be maintained on file.
The staff schedule was posted inside the Directors office which was not visible to educators, parents and visitors. Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors.
During am snack, the server did not wear gloves when handling food. The licensee must ensure that snacks and meals are prepared and served in a safe and sanitary manner.
Evacuation procedures were not posted at three out of the four exits. Next to each exit, emergency and evacuation procedures must be posted.
