The following toxic substances were accessible to children while at the Library. A container of Clorox wipes was placed on top of a table near the EEC board and entrance. A second container of Clorox wipes was placed on top of another table where two children played. A staff cabinet, used to store cleaning products and a staff's purse was kept unlocked and with both doors wide opened throughout the duration of the visit.
Staff records for five (5) staff were not available for Licensor review. The five staff folders noted the staff's name, however, did not contain documentation as per the Staff Records Checklist.

The Program Director use a check mark to indicate each child in attendance, noted the 3:33pm time on top of the sheet as arrival time for all children. Appropriate group size was not compliant as the daily attendance sheet reflected the maximum group of 45 children with 33 children in attendance at the initial start of the day. Technical assistance was provided.
One child's IHCP for {REDACTED} medication was not available for review.

Toxic Substances
At the time of the visit, (3) staff files hired within or before six months had no documentation of current first aid training. All educators must obtain within six months of employment, and must maintain thereafter current certification of training in basic first aid appropriate to the population served.
(1) staff file contained an expired CPR certificate with an expiration date of 9/18/2019. CPR training must be renewed annually.
(1) educator file did not contain BRC consent form. (7) educator files did not contain evidence of photo identification. BRC Program Administrators are required to secure a copy of the candidates' photo identification when submitting an EEC BRC. Evidence of completed background record checks, as required by 606 CMR 1.05(2) and 606 CMR 14.00;
The staff stated 12 children present at the time of the visit, however the daily attendance reflected 13 children in attendance. (1) child attending the program had not been signed in for the day. Attendance did not reflect children's arrival and departure times.

Staff Schedule/Qualifications not posted at the time of the visit. All educators must possess the qualifications required for their positions in accordance with EEC regulations. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors.
The Fire Inspection expired on 4/17/2019. The Building Inspection expired 9/1/2019. The program could not provide evidence of an updated Fire and Building Inspection at the time of the visit. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR).Fire Inspection. Every program not located in a residence must submit evidence of compliance with applicable fire codes.
At the time of the visit, the written emergency/evacuation procedures were not posted. Next to each exit, emergency and evacuation procedures must be posted.
A staff purse was placed on top of a rolling cart, accessible to children at the time of the visit. Educators must ensure that all hazardous objects, including but not limited to matches, lighters, toxic materials, sharp objects, plastic bags and purses are locked or inaccessible to children.
