The licensor reviewed 4 of the children records. The following were observed: 1.Three of the children records did not list the names of the staff (assistants and educator) on the general permission to be off the premises and permission to transport the children to a medical facility. 2.Three of the children records did not have a picture or a physical descriptions of the children.
