The program is licensed for a capacity of 39 children. Despite this licensed limit, daily attendance records showed that the program has exceed 39 children in attendance on several occasions.
The current Staff Schedule was not posted in a visible location.
The program's EEC License was not posted.
Children's Records, including emergency authorizations and contact information, were locked/inaccessible for approximately the first 15 minutes of the program day while children were in care.
A review of program documentation showed that monthly Staff Meetings are not being conducted/documented as required.
The program's Health Care Consultant Agreement showed that the consultant's Mass Certification is expired.
An expired {REDACTED} for an child that is no longer enrolled was still being stored at the program.
