At the time of the visit, the daily attendance reflected 26 children, however 24 children were present. Departure time for two children were not properly documented during transition.

In the playground area, a large climbing equipment which included three slides, and wall climbing structures was not supervised by staff upon Licensor arrival to the playground. Proper supervision was not provided at the Swing/Monkey bar area as one staff was observed swinging on the swings.
Emergency #'s were not posted in an area easily visible to parents, educators and visitors.
The emergency evacuation observations log did not include a practice fire drill for December 2019.
One staff's purse was placed inside a laundry basket on the floor in the cafeteria, well within the reach of children.
