At the time of the visit, two staff were out of ratio for a period of 14 minutes. (Two staff with thirty-four children were present in the cafeteria. Daily attendance reflected a general 3:00-6:00 PM time of arrival; however, children began arriving at 3:30pm. Educators did not include each child's arrival time on the daily attendance and used a check mark system.

This monitoring visit inspection recorded no violations or advisories.
The Building and Fire Inspections submitted in the Lead Portal Document Library were expired. Evidence of a Building and Fire Inspections were not accessible at the time of the visit.Fire Inspection. Every program not located in a residence must submit evidence of compliance with applicable fire codes.Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR).
At the time of the visit, (11) reviewed personnel records did not contain evidence of two references. (1) personnel record did not contain evidence of a program orientation, application/resume, physical and {REDACTED} records and {REDACTED} work verification form for a group leader. Documentation that the employee has the qualifications required under 606 CMR 7.09, including, but not limited to, health records, as required by 606 CMR 7.09(11).
At the time of the visit, the program's daily attendance reflected 30 children, however one staff stated 34 children while another staff stated 31 children present. Students present in the room was 28 at the time of the visit. Time of arrival and departure was not properly reflected on the attendance. Daily attendance records indicating each childs attendance, including arrival and departure times. Ensure adequate supervision of the group at all times.
At the time of the visit, (1) child's record file did not contain an Individual Health Care Plan for an {REDACTED} medication. (1) child's record file had an expired IHCP for an {REDACTED} medication which dated 5/15/2017.The licensee must maintain as part of a childs record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the childs health if the treatment is not administered.

(1) personnel record did not contain evidence of First Aid training for a staff hired within six months of employment. (10) staff records did not contain evidence of First Aid Training. All educators must obtain first aid training within six months of employment, and must maintain thereafter current certification of training in basic first aid appropriate to the population served.

The program had no documentation of individual written record for each child. A Children's Records Checklist was not available at them time of the visit. The licensee must maintain an individual written record for each child.
In the cafeteria space, Clorox wipes were stored in an unlocked cabinet which was accessible to children. Educators must ensure that all hazardous objects, including but not limited to matches, lighters, toxic materials, sharp objects, plastic bags and purses are locked or inaccessible to children.
