At the time of the visit two classrooms did not have an accurate attendance as the number of children on the attendance did not reflect the number of children in the classroom.

At the time of the visit the program did not have a BRC for one educator prior to working in the program and (2) educators BRC were expired.
At the time of the visit two educators were talking to eachother on the playground not supervising the children on the slide when a child walked up the slide and rolled down the slide backwards. The Director informed the educators of the incident.

The {REDACTED} classroom sink had the children's used drinking cups piled up from the day before as the classroom was not open on the day of the visit.
At the time of the visit the following medication was expired and/or missing: (1) {REDACTED} was expired and (1) bottle of {REDACTED} was missing for a child with a life-saving medication.
At the time of the visit the educator was wiping the children's nose and did not wash their hands or the child's hands in between wiping each nose.
