1. The program did not maintain a personnel record for a staff member working on site. 2. Documentation of an updated physical examination and the EEC Strong Start Essentials 12-hour training certificate were not available for review in two staff record files. 3. Documentation of a program orientation was not available for review in staff's record file. 4. Documentation of bi-monthly staff observations and an annual performance appraisal/staff evaluation were not available for review in a staff's record file.
The daily attendance did not reflect each child’s arrival and departure times.
