At the time of the visit, the program's emergency evacuation fire drill log did not reflect documentation of a May 2025 practice fire drill.
Staff monitor environment for hazards-Hazardous items not accessible to children
1. At the time of the visit, a staff's personal belonging (purse) was stored on top of an activity shelf, within reach of children. 2. At the time of the visit, children's prescription and over-counter medications were stored inside the first aid bag which was stored inside an open toy shelf, within reach of children.

At the time of the visit, cleaning products (Germicidal cleaners) indicating "Keep Out of Reach of Children" were stored in the cafeteria space, accessible to children.

At the time of the visit, a list of emergency numbers was not posted on site and nor available for review.
At the time of the visit, accurate attendance was not maintained as 19 children were marked in the attendance, however 18 children were present in the room and a staff member reported inaccurately 17 children present.
An educator was not adequately positioned to maximize their ability to see children in their care and was engaged in other activities that unnecessarily diverted their attention from supervising the children.
The Building Inspection expired 9/22/2024. The Fire Inspection was expired as of 8/15/2024.

At the time of the visit, a child’s prescription medication (Epi Pen) was not stored in the container/packaging in which they were originally dispensed and with their original labels affixed.

Storage space for children to store their personal belongings in a sanitary manner was no available as children's backpack were stored directly on the floor against a wall.
Building Inspection expired 8/27/2022. Fire Inspection expired 9/15/2022.
Emergency Evacuation practice fire drills for FY 2022 through June of 2023 were not available for review.
1. At the time of the visit, staff’s personal belonging (backpacks) were stored within reach of children. 2. Cleaning products (Clorox wipes) were stored within reach of children.
Supervision of Children
Toxic Substances
An adult size scissor was left unsupervised on top of a table, accessible to children. A staff’ purse was left half way opened on top of a table which was accessible to children. Electrical cords longer than 6 inches was accessible to children on the stage area.
The attendance log reflected five children in attendance, however six children were present at the time of the visit. The staff was aware of daily counts and had not documented arrival time for one child.

At the time of the visit, the staff's body was positioned facing a wall near a corner of the cafeteria which was facing away from some children that were running and playing around the stage area. As the staff paced around the room, the staff's head faced downward as they knitted a hat for a child, while four children ran around the stage area, two of the four children climbed on top of the half wall and sat on the poles.
On the date of the visit, the Building Inspection reflected an expiration date of 8/27/2022 and Fire Inspection had expired 5/22/2020 and dated 5/22/2019.
