The staff schedule was not posted and it was not current. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors.
Electrical wiring was exposed above the light switch in the bathroom located in the gym space area used by children.The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children.

The educator did not exercise appropriate supervision of a child who left the gym space area to get a toy from the storage closet when the Licensor was in the same storage closet.
Written Materials
A disinfectant spray was stored within reach of children in the game storage closet. The educators personal belongings where within children's reach in the gym area. Educators must ensure that all hazardous objects, including but not limited to matches, lighters, toxic materials, sharp objects, plastic bags and purses are locked or inaccessible to children.

The program's first aid kit did not contain a CPR mouth guard. The licensee must maintain adequate first aid supplies, including, but not limited to; adhesive tape, band-aids, gauze pads, gauze roller bandage, disposable non-latex gloves, instant cold pack, scissors, tweezers, thermometer, and CPR mouth guard.
The Building Inspection and Fire Inspection posted had an expiration date of 9/1/2019.
A copy of the health care policy was not available/posted in the program.
The program had no documentation of evacuation drills conducted for the months of April, May, June, September and October of 2019. The program did not have documentation for the effectiveness of the evacuation drills conducted.
One (1) child’s {REDACTED} medication was stored in a first aid kid in a storage closet. Medication did not have original label affixed and had an expiration date of 12/2018. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed.
Nine (9) children did not wash their hands before participating in the school breakfast program.The licensee must ensure that educators and children wash their hands with liquid soap and running water, using friction, in accordance with DPH guidelines. Hands must be dried with individual or disposable towels or automatic hand blow-dryers before eating or handling food.
One (1) child’s IHCP was missing from the child's file. The licensee must maintain as part of a childs record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the childs health if the treatment is not administered.
