1. Staff personal belongings (two purses/bags) were stored inside an unlocked closet space, accessible to children in the classroom. 2. Lotions and diaper rash ointments indicating "Keep Out of Reach of Children" were stored inside an unlocked cabinet and inside a drawer, accessible children.
This investigation inspection recorded no violations or advisories.

Handwashing procedures were not adequately conducted, as two children's hands were not washed following a diaper change.

A review of five children's records reflected the following. 1. Documentation of an updated developmental history form, transportation plan, and physical examination and lead screening results for two children were not available for review. 2. Documentation of a child's updated first aid emergency consent form was not available for review.
First Aid Certification for a staff member hired on 1/31/2023 was not available for review.
A review of four staff records reflected the following. 1. Ongoing bi-monthly staff observations were not available for review. 2. A staff record did not reflect an updated physical examination and a second staff record did not contain documentation of MMR immunization.
A staff record reviewed contained an outdated physical which dated 9/18/2013 for a recently hired staff. A second staff record did not contain documentation of a physical examination, and proof of {REDACTED} immunizations.
