Four rest materials (nap mats) were not maintained in good repair, exposing the interior foam material.
1. Appropriate handwashing procedures were not maintained as children did not wash their hands with soap and running water following toileting/diapering procedures. 2. A staff member did not wash their hands following diapering/toileting procedures.

1. Documentation of MMR immunization records were not available for review for a staff member hired on 7/22/2024. 2. Documentation of bi-monthly staff observations and staff information forms were not maintained in three staff record files.
A common changing table was used as a counter space to dry bins/toys.
At the time of the visit, staff’s personal belongings (lunch totes/bags) were stored within reach of children.

A child’s over the counter medication (Zyrtec) was found to be expired as of 7/2023.
1. At the time of the visit, the door from the HVAC/ Attic room located within a classroom space was unlocked and included a staff’s purse which hung within reach of children. 2. Electrical cords from a floor lamp and computer monitor were not safely arranged in a classroom space.

In the {REDACTED} classroom and extension cord contained multiple outlets which were not properly covered. Electrical outlets. All electrical outlets within the reach of children younger than {REDACTED} must be made inaccessible by use of a safety device or covering that prevents access to the receptacle openings. If the covering is a shock stop, it must be of adequate size to prevent a choking hazard. All electrical cords must be arranged so they are not a hazard to children. Electrical cords must not be frayed or damaged.
In the {REDACTED} classroom, (5) children's water bottles were not labeled with the child's name. In the {REDACTED} classroom, (1) child's water bottle was not labeled with the child's name. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
In the {REDACTED} classroom, a refrigerator thermometer reading was 68 degrees, the refrigerator appeared to be off or not working properly. In the {REDACTED}, the temperature reading was 46 degrees. In the {REDACTED} classrooms the thermometer reading was 18 degrees. The licensee must provide refrigeration and storage for food at not less than 32? F, nor more than 40 F for all food requiring refrigeration.
In the {REDACTED} classroom, paint peeling/flaking on the yellow 1/2 wall and blue wall. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster.
