At the time of the visit in the SACC program the staff records were not all current with the required EEC documentation.
At the time of the visit the following was not in compliance: The {REDACTED} program had (1) educator using their cell phone instead of supervising the children. In the SACC program the educators did not communicate with one another during the transition from swim to the bathroom for the children to change. As a result a child was left in the classroom and EEC alerted the educator about the child in the room next door.
At the time of the visit the SACC program sent the Group Leaders on break leaving the assistant leaders with children and without supervision.
At the time of the visit the following was not in compliance: {REDACTED} program did not have the child's ADHD medication in a locked cabinet, the prescription label was missing, the program was not administering the medication on a consistent daily basis, and the educators did not document when the child missed a dose. One child's medication was not located in the classroom with the child. In the SACC program (1) EPI pen was expired, and missing a bottle of Antihistamine.
At the time of the visit the following was not in compliance: The {REDACTED} program did not have the side effects listed, who was trained to administer the medication and who trained them The SACC program did not have (1) IHCP completed as the parent signature was missing and who trained the educators to administer the medication and who is trained to administer the medication.
At the time of the visit in the SACC classroom several pieces of furniture were not secured, a play kitchen set was broken with a screw sticking out, and the main outside door to both programs was not closing allowing anyone to enter the program.
At the time of the visit the SACC program did not have accurate transportation attendance logs as the children were not signed in or out accurately.

At the time of the visit the {REDACTED} program did not have evidence of the drivers and monitors with a completed BRC.
At the time of the visit the {REDACTED} program did not have all of the required documents in the educators file. The following was missing: (2) references for two educators, work verification forms, and evidence of diploma or degree for group leaders.
In the {REDACTED} classroom the screen needs to be repaired. In the {REDACTED} classroom the following needs to be repaired: the outlet cover is not flush to the wall, and the floor tile is missing.

In the SACC program the children arrived to the classroom and began having their snack without washing their hands.
The SACC program did not have enough equipment and materials for the amount of children enrolled in the program. The program needs to setup the space to meet the needs of the children enrolled in the program.
In the SACC: The children arrived at the program by bus and proceeded to enter the building and walk to the classroom (through two secured doors using a code) without the supervision of the staff.

In the Preschool classroom there were several educator purses, the chains on the shades are broken, and diaper cream in the bathroom that was within reach of the children.
