During the visit, the following was observed: INDOOR hazards accessible to the children were observed on the 1st floor level: 1) treadmill machine 2) Black fan that was plug in 3) Hammer on the floor near the back exit/sliding doors 4) box of screws on the floor near near the back exit/sliding doors 5) two white chargers on small side table in the living room area OUTDOOR space: 1) the backyard was partially fenced in. A section of the fence between educator and the neighbor's home was missing. The educator did not have a permanently installed barrier to prevent children from accessing the busy street in front of her home.

Per educator Person {REDACTED} has been regularly on the premises since last week (7/10/23). Educator has been using this person as an unapproved caregiver. Person {REDACTED} does not have a background free of conduct which, in the judgment of the Office, bears adversely upon applicant's or licensee's ability to care for children.

Progress Reports
During the visit, Licensor reviewed 5 records of the children that were present. The following was observed: -CHILD #1: developmental history was incomplete in the enrollment packet. Additionally, the child did not have a physical examination and immunization records on file. -CHILD #2: the enrollment packet did not have the transportation plan and schedule of days and times in care. -CHILD #3: physical examination was not up to date on file.
Space (indoor / outdoor hazards) - R Apr 2025
During the visit, the following was observed: 1) Educator stated Person {REDACTED} started to work in the program last week(7/10/23), however Person's {REDACTED} certificate expired on 6/28/23 and a renewal application has not been submitted. Additionally, Licensor reviewed children attendance record for the week of 7/10/23 to 7/14/23 and observed that Person {REDACTED} was used to meet ratios on Monday 7/10/23 and Wednesday 7/12/23 and educator had 8 children under {REDACTED} present.
During the visit, Licensor observed Person {REDACTED} feeding the {REDACTED} that was present in a rocking chair instead of holding the {REDACTED} in the lap as required. Additionally, it was observed that the {REDACTED} could not hold the bottle to feed by themselves.
During the visit, Licensor reviewed 5 records, including progress reports of the children that were present. The following was observed: 1) Four children (#1, #2, #3,and #4) did not have progress reports on file. 2) child #5 progress report was not up to date on file.
During the visit, Licensor observed there was no permanent barrier at top of stairways leading from the 1st floor to the basement. Additionally, educator stated the gate that was previously placed was removed.
During the visit, educator stated the program has only been using handsanatizer for educator and children to clean their hands, including after diapering/toileting.
During the visit, Licensor observed the educator has been using unapproved Person A to assist in the program since 7/10/23. Person {REDACTED} previously held a regular assistant certificate that expired on 6/28/23. Educator did not employ a Regular assistant who meet the qualifications for their respective position.
During the visit, Licensor observed a castle structure (Step 2 brand) with a green slide attached which measure more than 18 inches (30 inches) in height , and educator did not have a fallzone as required.
During the visit, Licensor observed Person {REDACTED} on the premises. Per educator this person has been regularly on the premises since last week (7/10/23). Educator did not have a method of knowing exactly who is present on the premises at any given point in the day as reqired.
During the visit, the following was observed accessible to the children on the 1st floor level: 1) Insect repellent can on kitchen island. 2) a purse on the small side table in the living room area. 3) stain blocking paint can and primer can on the floor near the back exit/sliding doors.
