Diaper ointment products were stored inside an unlocked diaper changing cabinet, accessible to children.

1. Two staff personal belongings (bags/lunch totes) were stored within reach of children in two separate classrooms. 2. A diaper rash ointment (Desitin) was stored inside a small 3-tier plastic drawer, accessible to {REDACTED} age children in a classroom. 3. A swifter mop containing liquid cleaning product hung behind a classroom door, within reach of {REDACTED} age children in a classroom. 4. Several cleaning products were stored inside the kitchenette area, accessible to children as the door was not safely secured.
1. A small nail was observed protruding from the top corner edge of a wood wall panel in a {REDACTED} age classroom. 2. Electrical cords longer than 6 inches from electronic devices were accessible to children in multiple classrooms. 3. Some peeling of paint was observed in two classrooms and on the hallway area of the top floor level. 4. Two restrooms were not closely monitored for cleanliness as several drops of urine were observed on a toilet seat cover and toilet paper and wrapper were observed on the floor. Toilet plungers were accessible to children. Various restroom doors and door frames were not properly cleaned, reflecting residue from paint and stains.

A diaper change was conducted directly on the floor of a {REDACTED} classroom in which the changing surface was not protected with a covering of adequate size to prevent the child from coming in contact with the changing surface/floor.
1. At the time of the visit, three staff's personal belongings (purses) were stored within reach of children in three classrooms. 2. A child's Epi Pen medication was stored within reach of children in a {REDACTED} age classroom. 3. An electrical cord longer than 6 inches from a bladeless fan was not safely arranged. 4. Two diaper rash ointment creams were stored within reach of {REDACTED} age children.

Electrical cords longer than six inches from electronic equipment and decorative lighting garlands were not safely arranged in classrooms and were found to be within reach of children.
1. Two children were not washed with liquid soap and water and dried following a diaper change. 2. An educator did not keep at least one hand on the child at all times while the child was being changed on an elevated surface.
