1. A cleaning product (Clorox Disinfecting Wipes) were stored inside an unlocked storage closet, within a classroom space 2. A liquid Wite-Out correction fluid indicating "Keep Out of Reach of Children" was stored on top of a desk, within reach of children.

1. Various indoor activity spaces were cluttered with furnishings and materials pressed against the walls. Materials blocking a hallway. Shelving (approx. 4 FT in height) on wheels appear to be tippable and not safely secured. 2. The lobby entrance area reflected significant peeling/chipping of paint on the windowsills, lower wall panels and framing. 3. A wall leading to the second floor- level, reflected a fist size crack on the sheet rock.

The outdoor facility reflected flaking/chipping of paint on the window and front door frame, on a side corner beam, and on the cellar door.
Documentation of a practice drill for the months of February 2023 through July 2023 were not available for review.
1. A staff's personal belonging (lunch bag) was placed on a countertop, which was accessible to children. 2. Plastic bags were stored in multiple areas of the kitchen, which was accessible to children. 3. Toxic/hazardous products (Kilz odorless oil base stain blocker, Kilz Latex and wall paint) were stored inside an unlocked cabinet inside the kitchen, which was accessible to children.

Staff orientation, training and meetings
at the time of the visit, the following was observed; 1. multiple licensed spaces/classrooms located on the second floor level appeared cluttered and used as storage. The {REDACTED} room on the first floor level appeared cluttered with materials/equipment shoved against the wall. 2. The program space did not have a barrier in place. 3. The flooring on the {REDACTED} contained a significant amount of peeling/flaking paint. 4. A bathroom located on the {REDACTED} floor contained peeling paint beneath the light switch. Additional peeling of paint was observed on the ceiling of a classroom located on the second floor level.

Documentation of a July 2021 and August 2021 staff meeting were not available for review at the time of the visit.
While outdoors, the time of arrival was not documented for six children upon each child's arrival time as six children arrived by/before 3:40pm, and their arrival time was documented on the daily attendance at 4:09pm, upon the re-entering of the facility.
The emergency evacuation fire drill observations did not reflect documentation of a practice drill conducted for the month of February 2021.
Indoor Space
Program space is in poor repair. Ceiling paint chipping in one room. In a room located on the 1st floor, the ceiling had a leak coming from the second floor bathroom which has damaged the hardwood flooring. One window in this room is broken and has been covered with cardboard, however the provider reporter that this room is currently not being used. Second floor level are not being utilized by children and have been used as storage space. Some children used spaces appeared to be cluttered while others spaces appeared to be too empty with very limited materials.
