At the time of the visit, diaper rash ointments (Desitin and Aquaphor) were stored on a shelf inside a restroom within the preschool classroom space, accessible to children.
1. Documentation of an updated physical examination was not available for review in three children’s record files, these were found to be outdated as of 7/17/2024, 5/26/2023 and 7/20/2024. 2. Documentation of a current lead screening was not available for review in four children’s record files.

Documentation of annual professional development trainings were not available for review in two staff's record files. 5. Documentation of a current physical examination and MMR immunizations were not available for review for a staff member hired on 1/13/2025. 6. Documentation of a program orientation was not available for review in a staff's record file.

At the time of the visit, two prescription medications (Epi Pens) were expired as of 11/2024. One of the prescription medications was not stored in their original packaging as the lot number and expiration date noted on the packaging did not match the lot number and expiration date noted on the actual medication.
This investigation inspection recorded no violations or advisories.

The medication lot number did not match the lot number noted on the original packaging.
1. A child’s Individual Health Care Plan from an Epi Pen medication was outdated as of 12/16/2022. 2. Two Children’s’ Individual Health Care Plans did not include parental consent.
