At the time of the visit, proper handwashing procedures were not followed as children were observed eating snack prior to washing their hands with soap and running water.

At the time of the visit, first aid certification was found to have expired on 10/21/2023. Through conversations with the administrator, first aid training has been scheduled for 4/28/2024.

At the time of the visit, CPR certification was found to have expired on 10/21/2023. Through conversations with the administrator, CPR training has been scheduled for 4/28/2024.
1. Two children’s records did not contain documentation of an updated physical examination (no appointment cards on file). 2. A child’s record did not contain an updated Developmental History form as of 8/28/2018. A second child record did not contain documentation of an updated Transportation Plan, First Aid Medical Consent form, Enrollment form and Developmental History form as of 2/28/2023.
