A child's bottle of [REDACTED] was found expired. Expired or no medication consent on file for 6 medications maintained for children at the program.
The following was found during review of IHCPs: 1. A child's IHCP expired on 9/21/2024. There were 2 different inhalers on site and no clarification which should be used when, 2. There was [REDACTED] on site for a child and the IHCP states only an [REDACTED]as part of the child's plan. 3. A child's IHCP expired 8/2024 4. No IHCP on site for a child with[REDACTED] and emergency medication
An assistant teacher was observed in a bathroom with a child that was divided by a half door and curtain, not allowing for direct supervision by a teacher at all times.

Dome climber does not have required 6 ft use zone around it and and it does not have the required 9 inches of an impact absorbing material under and around it.
During review of evacuation drill documentation it could not be determined if drills have been completed monthly. Documentation did not include the year the drills were conducted.

A child's first aid consent form was not signed by the parent.
During the visit the Licensor learned the program currently has 38 preschool age children enrolled but their capacity for preschool aged children is 36.
Program was utilizing a preschool classroom as a T/P grouping starting in September 2025. All children have aged up to preschool at the time of the visit. The program does not have a T/P grouping on their license.
Review of IHCPs found the following: 1 child's plan stated antihistamine, but did not state which one, 1 child's plan stated an [REDACTED]but there was no inhaler at the program, 1 child's plan stated [REDACTED] as part of their plan and there was none on site, and 1 child's plan was expired.
Not all medications at the program have a medication authorization form signed by the child's parent.

There were 2 medications at the program with no attached doctor's orders. There were 3 medications with no medication consent on file.

An educator who was not teacher qualified was conducting the program's ongoing covid testing. The educator was observed alone with children.
Program did not have current medication consent forms on site for all children's medications. One child's medication consent stated a different [REDACTEDdosage than the child's IHCP.
A child's IHCP was found expired. One child's plan states [REDACTED] as part of the emergency plan and there is no [REDACTED]on site and this plan did not state an actual [REDACTED] dosage.
